Rocky Mountain PACE: supporting independent living for seniors 

Many older adults have a goal of remaining independent and aging in place. Yet, as health needs change, managing appointments, medications, transportation and daily help can overwhelm both seniors and families. Rocky Mountain PACE bridges this gap with a compassionate, all-inclusive care model that empowers older adults to stay safe and healthy at home while remaining connected to their community.

Rocky Mountain PACE is a nonprofit with a clear mission: to empower seniors to live well and age with dignity. It serves adults 55 and older who qualify for nursing-level care but want to stay home. The PACE program offers an alternative to long-term care facilities. 

A team built around the individual

Rocky Mountain PACE stands out with its team-based, relationship-driven approach. Each participant has an interdisciplinary team consisting of their primary care providers, therapists, social workers, nutrition specialists, transportation specialists, and others — who work together to meet the individual’s needs.

PACE takes care of the scheduling and maintenance of health care for participants in their program. This integrated approach reduces stress, closes communication gaps and ensures proactive care. As needs change, care plans adapt swiftly and thoughtfully, always with participants’ best interests in mind.

Comprehensive services under one roof

Rocky Mountain PACE has three locations in Colorado Springs with clinics and day centers. Here, participants meet with medical providers, receive therapy, enjoy meals, participate in activities and connect with peers. Transportation to and from the day center keeps participants active and engaged.

PACE services include —

  • Primary and specialty medical care tailored to each participant
  • Physical, occupational and speech therapies to support mobility and independence
  • Medication management and pharmacy services to ensure safety and accuracy
  • Transportation to medical appointments and the day center
  • Social activities, meals and recreational programs that promote mental and emotional well-being
  • Care coordination and caregiver support to help families navigate changing needs

This all-inclusive model ensures participants get the right care at the right time,  without managing multiple providers or complex insurance. For many families, PACE brings peace of mind and stability.
